Appointment Reminders

Please remember to bring your insurance and prescription cards as well as a current list of your medications.
We participate with most major insurance carriers. As a courtesy, claims will be filed for you. In order to ensure reimbursement, your insurance information must be kept current. Please be sure to notify us if you have any change in coverage or insurance company.

If you cannot keep your scheduled appointment, please call at least one day ahead to reschedule. Because your treatment schedule is important to your care, call as soon as you know you will need to reschedule. If you need to reach a physician after hours, please call your regular treatment facility for the physician on call.
